Insurance

What insurance plans do you accept?

We accept most major insurance plans, including Regence, Premera, Blue Cross Blue Shield, Cigna, Aetna, and Medicare. Coverage varies by provider and plan. Call our office before your appointment to verify your specific plan is accepted and understand your expected out-of-pocket costs. If you have a different insurer, feel free to check on your insurer's website or give us a call.

Will my visit be covered by insurance?

Medical dermatology visits — skin checks, biopsies, and treatment of conditions like acne, eczema, or psoriasis — are typically covered subject to your deductible and copay. Cosmetic procedures (Botox, fillers, CoolSculpting, laser hair removal) are not covered by insurance and are billed directly to the patient.

Do I need a referral?

A referral is generally not required for cosmetic procedures. For certain medical dermatology services, a referral may be necessary depending on your insurance plan. If you're unsure, call your insurance company or our office before scheduling.

Understanding Your Bill

What is a deductible and how does it affect my bill?

A deductible is the amount you pay out-of-pocket before your insurance begins covering costs. If you haven't met your annual deductible, you may be responsible for the full allowed amount of your visit. Once your deductible is met, you typically pay only a copay or coinsurance percentage.

Why did I receive a bill after my insurance paid?

Most insurance plans require patients to pay a portion of covered services — called a copay or coinsurance. You may also receive a bill if your plan applied the charges toward your deductible. The amount billed to you reflects what your insurance determined is your responsibility after processing the claim.

Why am I being billed twice for my surgery?

Surgeries performed in our Ambulatory Surgery Center generate two separate claims. The provider (Dr. Levy, NPI 1427053834) bills for the professional component, and the ASC (NPI 1902043284) bills for the facility component. Both are legitimate charges for the same procedure and are submitted to your insurance independently.

Why am I being billed for a procedure my insurance said would be covered?

All claims are ultimately subject to your specific insurance plan benefits. You are responsible for your deductible, coinsurance, and copays regardless of what was discussed prior to your visit. If you believe your benefits were processed incorrectly, contact your insurance company directly to dispute the claim.

Payment Options

How do I pay my bill?

You can pay online through our patient portal, by phone during office hours, or in person at either office. We accept major credit and debit cards, cash, checks, and CareCredit. Same-day payment is appreciated; balances are due within 30 days of your statement date.

Do you offer payment plans?

Yes. If you're unable to pay your balance in full, contact our billing department to discuss a payment arrangement. We also accept CareCredit — a healthcare financing option that can spread payments over time with promotional interest-free periods for qualifying purchases.

What if I have a question about my bill?

Call either office and ask to speak with our billing team. Have your statement and insurance card handy. We're happy to walk through your charges line by line — no question is too small.

Your Appointment

What should I bring to my first appointment?

Please bring a valid photo ID, your insurance card, any relevant medical records or prior dermatology reports, and a list of your current medications. If you have photos of a skin concern that comes and goes, those are helpful too.

Are there age restrictions for cosmetic procedures?

Most cosmetic procedures are available to adults 18 and older. For patients under 18, parental consent and a consultation are required before proceeding.

How do I prepare for a cosmetic procedure?

Preparation varies by procedure. Generally, you'll be asked to avoid certain medications and sun exposure before treatment. Specific instructions will be provided at your consultation. Call our office if you have questions before your scheduled date.

Our Offices

What are your office hours?

Both locations are open Monday through Friday, 8:00 AM to 5:00 PM. We are closed on weekends. Same-week appointments are often available — call to check current availability.

Is parking available?

Yes. Both the Burien and Bellevue locations offer free, spacious parking. The Burien office is also handicap accessible. The Bellevue office is located near Whole Foods on 116th Ave NE.

Your Rights

What is the Balance Billing Protection Act?

Washington state and federal law (the No Surprises Act) protect patients from unexpected out-of-network bills in certain situations. If you receive care you didn't expect to be out-of-network, you have rights under these laws. Contact our billing department if you have questions about a bill you believe may be covered by these protections.
