Telehealth Registration

Welcome to Dermatology of Seattle & Bellevue Telehealth

We are excited to be able to offer this service to you as it helps conserve valuable PPE and limits exposure to one another.

Our office is happy to assist you with the process. Please reach out if you get stuck along the way

Step 1

Complete & sign our registration form

Our secure registration form includes your info, emergency contact info, and acknowledgement of our privacy practices.

Step 2

Upload a copy of your ID & Insurance card

You may scan them and upload from your PC or take a clear photo using your Apple or Android device. Please ensure that we receive the front and back of the insurance card.

Step 3

Activate your patient portal

A separate email was sent with instructions after your appointment was scheduled.  If you do not see this email please check your junk and spam folders.  The activation link expires after 48 hours.

Step 4

Download the app

Download & install the PocketPatient™ app on to your mobile device.

*Android Users: Under the portal settings tab there is a link to change your password.  Your preset password is Password1.  You will be asked to change it after you log in.

Step 5

Fill out your medical history and medications

Complete the My Health sections 1-6 in your patient portal.

As a courtesy our office will call you the day of your appointment to verify you are set and ready for your telehealth visit.

 

Additionally, we recommend bookmarking your patient portal. https://dermatologyseattle.ema.md/ema/PatientLogin.action 

The patient portal allows you to review chart notes/labs/pathology, email staff/providers, request appointments and prescription refills.
